Registration Requirements (Download a packet below).

Registration must be completed in person by the student’s parent or legal guardian. The following documentation is required at the time of registration:

• Student’s birth certificate
• Health records, including proof of immunizations
• Prior school records, including transcripts, report cards, and any 504 Plan or Individualized Education Program (IEP)
• Residency verification (see below)

Residency Verification

In accordance with District policy, all parents and legal guardians must provide current residency verification annually for each student enrolled in the East Providence public schools.

Acceptable documentation includes a current lease agreement or mortgage statement. The residential address must be clearly visible on the document submitted.

Families who do not have a lease agreement or mortgage statement — including those residing with extended family — must complete a Residency Affidavit. This legal document is executed by the owner of the residence and confirms that the parent or guardian and student(s) currently reside at the stated address.
